Medical professionals have struggled to agree a definition for premature ejaculation - not entirely surprising given the variability in both ejaculation times and how long partners want sex to last.

To qualify as a medical condition, premature ejaculation generally means coming within less than 2 minutes of sexual stimulation. If this results in the man feeling distressed and is affecting his relationship with his partner, then it is right to seek medical help.

Symptoms

  • Ejaculating in less than 2 minutes of stimulation on a regular basis
  • Lack of control
  • Distress and lack of sexual satisfaction

Causes

  • Psychological factors such as: depression; stress; unrealistic expectations
  • Performance anxiety
  • Interpersonal dynamics eg hurt feelings; lack of emotional intimacy
  • Physical – low levels of serotonin

Available treatments non-medical

  • Anaesthetic cream, condoms or sprays - can be used to remove sensation from the penis and are effective in delaying ejaculation. Condoms with Lidocaine inside them are available as are sprays to apply to the penis. Emla cream contains Prilocaine and Lidocaine – can reduce sensitivity for your partner as well so you risk less enjoyable sex
  • Sex therapy – techniques can be learnt to reduce arousal eg squeezing the head of the penis or stopping stimulation before ejaculating – can be effective but obviously embarrassing if therapists involved
  • Counselling – psychologists can help some men to learn to gain control over their ejaculation.- can be expensive but added benefit of dealing with any associated depression or anxiety
